Welcome to our web site. Dr. Hamilton is a
Board Certified Family Physician, Kristi Chase is a certified Family Nurse Practitioner,
and Dahlia Maskin is a certified Physician Assistant, who practice in the North Phoenix/Scottsdale
area. Our goal is to provide you
and your family with quality care in preventing, diagnosing and treating
your health related problems.
We
aim to become your "medical home". We
see patients of all ages.
Our office is in the Bell Professional Center
which is conveniently located at the south-west corner of 55th Street and East Bell
Road in North Phoenix. We are accepting new
patients. There is
an on-site digital X-ray facility, and lab for your convenience.
On the Forms page you will find registration, health history and
record transfer forms that can be downloaded and completed prior to your
visit. Current patients will find a record release form to download and fax
in to have their records transferred from my previous group office.
