It is important that you understand what is and isn't covered by your health insurance policy. There are many terms such as co-pays, coinsurance, deductibles, and excluded services that have an impact on what you are required to pay for yourself. The doctor does not decide what is covered or not, it is governed by the terms of your health insurance policy. If there is something that is not covered then read the explanation of benefits (EOB) that is sent by the insurance company (Medicare Summary Notice if a Medicare plan) or contact the insurance company directly for an explanation.
