Our office is conveniently located in the Bell Professional Center on the south west corner of 55th Street and East Bell Road. There is ample free parking, we are wheelchair accessible and it is on a major bus route.

Our office hours are 7:30 AM to 5 PM Monday through Friday. Same day appointments are usually available for acute illnesses. There is a provider on-call after hours for emergencies. Complete and bring your registration forms with you - click here.

We accept most insurance plans including Blue Cross Blue Shield, United Health Care, PHCS, CCN, Mayo, CIGNA (HMO and PPO), Humana PPO, Arizona Foundation, Great West, Lifewise, Aetna PPO, Pacificare PPO, Worker's Comp and Medicare.

Services

Complete physical exams, PAP tests and well child exams.  School, camp, and sports exams  Immunizations including Tetanus, Gardasil, Shingles, Meningits, Pneumonia and Flu Shots.  DOT (CDL) exams  Workers compensation care  ECG's  Spirometry  In office surgery (biopsy and removal of skin lesions, ingrown toenail removal, laceration repair)  Blood tests and X-rays are performed on site for your convenience - no need to travel anywhere else.
